He's the dolly grip, which explains a lot.

What most people don't know is that the term originated with the filming of "Hello Dolly". Barbara Streisand was allergic to a lot of those feathers that were in her costume hats. The poor woman was constantly congested, and with that beak of hers, she flowed like Niagara falls. To make matters worse, she had next to no mobility in the dresses she was wearing, and certainly no place to tuck a tissue. You see where this going don't you? Yep, that's right; someone had to "grip" Dolly's big ol' massive snout with a Kleenex and let her blow. Hence the origin of the name "Dolly Grip"

Now as the crew moved from picture to picture, the name stuck. Dealing with a lead-actress diva became so common, that some poor bastard had to be the attendant to the basic needs and whims of the actress, some of which could be just plain gross and disgusting. So the term "Dolly-Grip" stuck. I hear that there is some pretty horrific stuff concerning tending to Betty White's carnal needs.

Now, the term "Best-boy grip"; well, that all started with Kevin Spacey movies...

Well now, since there exists a Best Boy grip, that also implies that there is a Good Boy grip and a Better Boy grip.
