So here's a couple better pictures. The shower and throne on one wall with the sink on the adjacent wall. Across is the furnace, water heater and soft water tank and a door on either end, one to an extra bedroom and one to a family room.

The one scenario that makes sense to me is pull out the shower and remove the sink. Put a single vanity and sink in the place of the shower and turn it into a half bath which would at least be usable. I still have about 1,000 sq feet of unfinished basement so I could build a full bath adjacent to the existing bedroom. Thoughts?

If you could move that wall and put the pedestal or vanity on the same wall as he toilet it would be better. Your idea of removing the shower and putting the vanity there is also good. Either way you will have to bust out a little concrete for the drains.

No worries about those crack in the slab. I'm sure the contractor who built it knew what he was doing and all. Ask him- he'll tell you so.

Basement slabs crack all the time. You only really worry when it’s wider than I like 1/2” or something.
