Idiots. This is why I never worry about paying balances to insurance companies. Their shit is rife with errors. I would be comical to see them try to prove anything in court. In this case they over paid the provider.

I also recently had a hospital get denied because they ran an old insurance number. I provided them with my new insurance card, which they photocopied and still filed it wrong.

Plus BlueCross is still billing us for a policy we cancelled (by phone and in writing) as of March 1st. They said we may get some bills but to ignore them.

Even dumber, we replaced that policy with another FROM THE SAME COMPANY.

Absolute chaos. My wife was getting all freaked out and was going to proactively try to unravel it, but I finally got her to understand that it's not our responsibility to supervise and correct their billing errors.

We cancelled the policy.
We purchased a new policy.
We paid the new premiums
We gave the correct information

The rest is their problem.
