A similar thing happened on the train coming back from the 4th of July fireworks in downtown Chicago years ago. A kid from my area that some of my friends knew was sticking his head and part of his body out of a window on the train. It was an awning window so it opened from the bottom like this |\. At one point there was a telephone pole right next to the tracks and the third time he did this BAM! He was dead instantly as the top part of his head was gone. A buddy of mine who's a fireman was there and saw it.

Happened here a few years back. Two drunk yeehaws in a pick up went off the road. Passenger had his head sticking out the window yelling at some girls. Driver missed the telephone pole, but the passenger got caught and beheaded by the guy wire anchored to the ground.
