Where the barrels came off the revolvers, good chance they were reloads that were "too hot" ( loaded to too high a pressure --wrong powder or too much powder)

An obstruction in the barrel can make it happen ( like where the shotgun came apart )

Towards the end, where the guy keep recocking the rifle by moving the bolt, that was a "hang fire". It probably "sparked enough" on one of the trigger pulls to get the powder smoldering and when it finally built up enough pressure to move the bullet, it ended up with too much pressure.

Sometimes the firearm itself is to blame.

When doing enuff of your own reloading, shit happens, no matter how careful we try to be.....I had a gun/reloading hobby back in the later 60's-early 70's and I used to reload a ton of ammo and among my favorite cals was of course the .44 mag.....8 3/8, and 10 5/8, one was nickel plated, rosewood grips...smooth sided.....

one time the gun went off, and the bullet stopped in the barrel about 1/2 way down, not enough powder I suppose....I went to shoot it again, earmuffs on of course, and he shouted SUPER LOUD to not pull that trigger, good thing or we be dead.....he was standing next to me.....out in a cinder block barn on the property of the Potomac Electric Power Co., part of the utility that supplied all the power to the Md. region and much of DC......another time out there I was in the driver seat of my '67 Grand Prix....and went to unload my .44 so thought it was unloaded and was discussing grip and trigger pull aiming toward the trans tunnel he saw the cartridge come around and said, 'DO NOT pull that trigger.....it would have been a LONG walk home, deaf as stones.....
