First of all I'd want to make it look like an accident. People deal with that better than suicide and it will minimize mental anguish for those I care about. Something along the lines of Vanishing Point would be fitting for me.

It's easy to speculate when you are not in the position. I don't want to be a burden in the end. I think when people start losing interest in my condition, it is best that I go.

That pain you're referring to only lasts a short time. Maybe 10 minutes or so. I've felt that pain more times than I can count and crossed the threshold into hypothermia on several occasions.
