Where the barrels came off the revolvers, good chance they were reloads that were "too hot" ( loaded to too high a pressure --wrong powder or too much powder)

An obstruction in the barrel can make it happen ( like where the shotgun came apart )

Towards the end, where the guy keep recocking the rifle by moving the bolt, that was a "hang fire". It probably "sparked enough" on one of the trigger pulls to get the powder smoldering and when it finally built up enough pressure to move the bullet, it ended up with too much pressure.

Sometimes the firearm itself is to blame.
