A: The masses who buy airline tickets based on one thing, and one thing only; low price. Airlines are simply giving most people what they demand. If people really cared about those things you list, Spirit, Frontier, SouthWest, Allegiant and all the other "discount" airlines wouldn't exist, yet they're always packed full.
If you're willing to pay more for First/Business Class, you still get large seats, meals, more leg room, better service, etc. Buy cheap tickets, get cheap service.

The fault lies in the airlines figuring out they can add 20 more seats and soak more money out of a trip they were already making.
Adding flights - EXPENSIVE
Serving food - EXPENSIVE
Adding seats - FINANCIALLY REWARDING
Removing meals - FINANCIALLY REWARDING
If every flight is $1500.00, then a $1200.00 flight is cheap and would attract more people. The entire fare structure was destroyed by Southwest.
