I've had those issues on both shoulders, including frozen shoulder. Not fun.

With my right shoulder, I went through the injections and PT, but after 6 months with very little relief, went for the surgery to repair a bone spur and a tear. When the second started acting up about a year and a half later, the doc told me I could do the injections and PT, but because it felt just like the other shoulder had, I immediately chose to do surgery. The left shoulder was a little worse. Post surgery PT was absolutely critical, and recovery took about a year with both of them, but I'm back to about 98%. Even though it's been about 10 years ago, I still continue to do the PT exercises.
