I don't know how the big boys do it, but when I go into a bad area I always shut down and lock the cab, and I never go where I can't see the entrance points to the box. Being non-emergency we don't carry any narcs on the truck, but they see the ambulance and think they can score something.
As far as being harassed, I just keep driving and don't slow down.
