One of the big fundamental flaws with so many of these developers is that they NEED to implement encryption at rest, meaning when it's just sitting in the database. The project that I'm on is with a financial services client, and encryption is an extremely high priority. There are tools to assist with implementing this encryption, but it doesn't just come out of the box.
Lotta fly-by-night outfits with 20-somethings doing development. You get what you pay for.
BUT, I agree that SF should have prevented these attacks in the first place, but there is a definitely an extra layer of protection that can be implemented by those that know what they are doing.
