What determines when a movie moves from the theatre to DVD/Blu Ray? Is it driven by a certain gross, or ranking?
What determines when a movie starts showing at less and less screens?

For the most part a film's run is scheduled, these days around 6 weeks. If a film blows up a hit, they'll extend, if it flops it starts losing screens almost immediately.
The release date for the DVD/BRD is determined by the length of time needed for creation of any extra features for the release, and then targetting a Tuesday in a high sales quarter without any other high profile releases.
The two are exclusive, a film can still be playing in second run theaters and available on DVD/BRD.
You can pretty much expect that anything in theaters before Labor Day will be available on DVD/BRD no later than Christmas Day.
